The U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) has confirmed that the Voluntary Self-Identification of Disability Form (CC-305) remains an OMB-approved form, and the current version has been approved for use through July 31, 2029. The form is used by federal contractors to comply with disability self-identification requirements under Section 503 of the Rehabilitation Act.

Form Remains Required for Covered Federal Contractors

Federal contractors and subcontractors subject to Section 503 are generally required to invite applicants and employees to voluntarily self-identify disability status using the approved CC-305 form. The form assists contractors in measuring their outreach and affirmative action efforts relating to individuals with disabilities.

The Department of Labor’s webpage continues to provide the approved form in English and multiple additional languages for contractor use.

Limited Modifications Permitted

OFCCP reminds employers that the CC-305 remains an OMB-approved form, and contractors generally may not alter the form’s content.

According to the agency, the only portion contractors may modify or remove is the “For Employer Use Only” section, which is intended to provide flexibility for internal recordkeeping and data analysis purposes.
